If you are out of the State you may use these forms but you will need to find your own online connections to help build healthy communities.

Youth Helping Youth - Help locate the hidden resources and services for youth at risk. Homeless youth are on the rise, and there are few community resources to help. Call your local agencies, faith based organizations, neighborhood groups to see what services they can provide.
